Arizona, California, Colorado, Michigan, New Jersey, Virginia, Connecticut, New York, Illinois, Texas, Florida, Pennsylvania, Missouri, Vermont, North Carolina and Maryland

If we can work together via your insurance plan, your visits will likely be fully covered (yes, free) or you may need to pay a copay per visit. That’s it! The majority of my clients pay nothing out of pocket to work with me! I am based in Clinton, Connecticut but I work with clients virtually all over the country!

My goal is to teach you the tools and skills you need to keep any results you achieve. I will meet you where you’re at and we will go along on your journey with a route we both agree upon. We will tweak as we go. I don’t have a one size fits all plan. Some people like to track their food, some are not open to it. 

My goal is that you feel comfortable with me. Laugh along the way. I’ll be by your side through the good, bad and the ugly. I will meet you where you’re at and we will come up with a plan that is individualized to you and your needs. There is no one size fits all plan with me. I approach each individual differently.

I’ll provide you support, accountability and be your cheerleader on your journey. We will work together as a team!

How to contact your insurance company to check your benefits as well as whether your plan will cover our time together

Well Balanced with Brooke by Brooke Rosenfeld accepts and is in network with: Anthem, Blue Cross and Blue Shield, Aetna, Meritain Health, United Healthcare, Oxford, NYSHIP, UMR, Cigna and Medicare insurance plans.

If you are covered by another insurance company, I am more than happy to provide you with a superbill to submit to your insurance company for reimbursement for our sessions. The superbill will not guarantee reimbursement.

Call the member services number on the back of your card and ask:

1. Is Brooke Rosenfeld considered in network with my plan? (NPI 1891392585)

2. Does my plan cover outpatient nutrition counseling? (codes: 97802 and 97803)

   • If yes, how many how many sessions/visits are allowed per year? Does your plan have unlimited visits or do you have a limit per year?

   • Does my plan cover nutrition counseling as a preventive service? (if you are overweight or obese, your insurance will likely cover visits as preventative)

   • Does my plan cover nutrition counseling to treat medical conditions such as diabetes, hypertension, heart disease, chronic kidney disease, ulcerative colitis, irritable bowel disease, etc

3. Does my plan cover telehealth for nutrition counseling? (location code: 10)

   Does my plan cover telehealth for providers located out of state?

   Do I have a deductible to meet first? 

   • If yes, how much is my deductible? 

   • How much of the deductible have I met?

4. Do I have a copay for outpatient nutrition counseling per visit?

5. Do I need a physician referral or a prior authorization?

7. Record the representative’s name and a reference # when checking your benefits. This information will be necessary if you ever need to dispute a rejected claim.
